Criteria:

The Public Health Service Isolated Hardship Award is an award of the Department of Health and Human Services that is presented to PHS officers stationed at remote posts. Their service must be for over 180 consecutive days at a site designated as isolated, remote, insular or constituting a hardship. Service aboard a ship that is absent from its home port for longer than 180 days qualifies. Ready Reserve officers qualify for the award if they complete an total of 180 days served within a 3 year period. While it is considered a civilian award it is approved for wear on all military uniforms.
